Application Documentation

(2) Completed BATFE FORM 4 (5320.4)

BATFE FORM 5320.23 (Required if using a NFA Gun Trust)

(2) Passport Photos

(2) FBI FORM FD-258S Finger Print Cards

$200 Check to BATFE-NFA


NFA Tax Stamp Filing Instructions

Carefully complete (2) BATFE Form 4s (5320.4) and sign in blue or black ink

Carefully complete duplicate copies of BATFE Form 5320.23 if using a NFA Gun Trust

Obtain (2) passport photos and affix one to each Form 4; do NOT staple

Make (1) photocopy of your completed Form 4 with passport photo affixed

Find local approved entity to complete (2) FBI Form FD-258S Finger Print Cards 

Write a $200 check to the BATFE-NFA

Identify your local Chief Law Enforcement Officer (CLEO)

Mail your completed paperwork to the BATFE and your CLEO

Once your application is complete you will receive an approved Form 4 which is everything you need to complete the transfer of your NFA Restricted Item
